The goal of predictive analysis is to identify populations who are, after 30 days, at high risk of hospital readmission. My expertise is critical in identification of these patients at Stony Brook University Hospital and other hospitals in Suffolk County. Our team is developing, testing and deploying predictive models that will identify these high-risk patients. Doing that will also help identify people who could benefit from enhanced clinical and support services. Also, transition-of-care health care professionals will be better served when we know who is at risk of hospital readmission.

 

One of the goals of the Delivery System Reform Incentive Payment program is to reduce, by 25 percent, the county-wide 30-day hospital readmission rate. We are analyzing and creating better clinical features in electronic health records systems. These engineered features will be used to improve algorithms that predict and prevent hospital readmissions in the DSRIP population.
